Email Policy

Communication with our practice through our website and/or via email is not encrypted and is therefore not secure.  Use of the internet and email as a method of communication is for your convenience, and by using this form of electronic communication, you assume the risk of unauthorized use.  By initiating contact via email or providing your email when scheduling/filling out paperwork you agree to withhold Allen Aesthetic Surgery, PA, its doctor, medical staff and affiliates, harmless from any unauthorized use of your personal information by outside parties who may steal this information by unlawful means.  If you prefer not to receive communication via email, please notify the practice.

Payment Policy

We require payment in full at the time services are rendered.  If you have any questions about your fees or charges, please ask prior to treatment.  For information on the forms of payment we accept and financing options we suggest please contact us.

Skin Care Return Policy

All of the skin care products we carry are considered pharmaceutical grade and are physician exclusive.  Just like any other prescription from a physician or pharmacy, we cannot return or refund products once they have left our office.  We make every effort to match a patient with the right skin care product(s).  Dr. Allen and her staff will assist you with selecting the right products for your skin care goals, but we cannot guarantee complete satisfaction with your purchase.  Follow-up appointments and phone are available to guide you in the process of medical skin care.

FINANCIAL POLICY

Fees

All fees for cosmetic surgery and/or treatments are payable in advance by cash, personal check, or credit card. We do accept VISA, MasterCard, and CareCredit for your convenience. Credit cards are subject to a 3% convenience fee. There is a $35 fee assessed to all returned checks and forfeiture of any discounts applied to the initial transaction for any returned checks.  FULL PAYMENT is due on the day of service. Total cost of a procedure will be discussed prior to treatment. No refunds once treatment has been performed.

Consultation Fee

A consultation fee of $100 is due at the time of booking your appointment. Dr. Allen spends the time necessary to answer your questions and ensure you are at ease prior to making a decision. For your convenience, our office honors Visa, MC, Discover and Amex. Your personal surgical quote is given at the conclusion of your appointment. Surgery quotes are honored for 3 months from the date of issue. Patients that do not schedule a surgical procedure within 6 months of initial consultation are required to have a 2nd consultation with additional fee payment.

To cut down on the number of “no-shows”, first time injectable or laser patients are charged an initial $100 consultation fee which reserves your appointment time.  This fee is applied towards treatment IF you schedule the treatment at your initial visit. Treatment must be done within 6 months of initial consultation for the fee to be applied towards treatment cost.

Surgical Fees

When booking your cosmetic surgery, 10% or $500 (whichever is greater) NONREFUNDABLE surgery deposit is required.  The deposit is applied to the balance of the surgery.  Full Payment for cosmetic surgery is due in FULL at least two weeks before the scheduled surgery.  There are 2 major components that add to the cost of a surgery: the surgeon’s professional fee, and the surgery center fee (which may/may not include: anesthesia, and/or operating room supplies.  Pre-operative testing, physical, radiographic imaging, etc are NOT included in the cost of the procedure. Extended surgery time due to unforeseen circumstances or an unusually extended time period for recovery add additional expenses not included in the surgical quote.

The patient is responsible for obtaining the necessary testing prior to surgery. The completed forms must be sent to the office for review two weeks prior to the scheduled surgery date to ensure the case proceeds as scheduled. If the necessary paperwork is not received in time, the patient will be subject to a 35% rescheduling fee.  If you choose to pay your surgical balance with a credit card, an additional 3% convenience fee is collected.

Cancellation/Rescheduling Policy

Office visits/consultations: Please note 24 hours notification is required for office cancellations so that other patients can be placed in that time spot. You will be given a full refund on your consultation fee if you notify us at least ONE business day (24 hours) ahead of your scheduled consultation. No refunds will be given for same-day cancellations or no-shows, and fees paid will not apply as a credit for future treatments. An additional non-refundable consultation fee shall be required prior to booking subsequent appointments.

Surgical procedures: Full payment is due 14 days prior scheduled procedure date. Once you schedule your surgery date, we ask you make every effort possible to avoid cancelling or re-scheduling.  However, if it becomes necessary to cancel or reschedule, any payments (minus the non-refundable surgery deposit) are refundable up until two weeks before the day of surgery. A rescheduling fee of $1000 will apply if rescheduled within the 2 weeks and 35% fee (of total cost) will apply if rescheduled within 7 days of scheduled surgery to cover the inconvenience of surgical change with staffing. The initial deposit will NOT be applied towards total surgery cost if rescheduled. Exceptions may be made for documented medical emergency or disability.

If you cancel or reschedule your surgical procedure, the cancellation fees are as follows:

8-14 days prior: 35% of the total surgery fee becomes nonrefundable
2-7 days prior: 60% of total surgery fee becomes nonrefundable.
Within 48 hours of scheduled procedure: 75% of the total surgery fee is nonrefundable.

Cosmetic Surgery Policies

The completed forms are to be sent to Dr. Allen two weeks prior to the scheduled procedure to ensure the case proceeds as scheduled or the patient will be subject to a %35 rescheduling fee.  If you choose to pay your surgical balance with a credit card, an additional 3% convenience fee is collected.

Cancellation Policy

Full payment is due 14 days prior scheduled procedure date. Once you schedule your surgery date, we ask you make every effort possible to avoid cancelling or re-scheduling.  However, if it becomes necessary to cancel or reschedule, any payments (minus the scheduling fee) are refundable up until two weeks before the day of surgery.  If the surgery is rescheduled within two weeks of the procedure date, 50% of the total fee is credited back. If you cancel your procedure, the cancellation fees are as follows:

8-14 days prior: 25% of the total surgery fee becomes nonrefundable
2-7 days prior: 50% of total surgery fee becomes nonrefundable.
Within 48 hours of scheduled procedure: 75% of the total surgery fee is nonrefundable.

Insurance

Please note Dr. Allen does not participate in insurance plans.  You are responsible for all costs related to your surgery/procedure should you choose to do so.  If you desire to submit a claim to your insurance provider, we can provide the diagnosis, CPT codes, documentation, etc for you to file the paperwork yourself.

REVISION POLICY

As with all surgical procedures, cosmetic surgery involves certain risks, limitations, and the possibility of complications. We seek to obtain the best possible surgical result, but there is no absolute guarantee or warranty as to the final result that may be achieved. Surgical fees are paid for the performance of surgery and not for the realization of an exact anticipated result. Although unusual, revisionary surgery is sometimes necessary in about 5-10% patients.

Every patient should understand that revision surgery will only be done after a full year from the surgery date. It takes that amount time for the tissues to fully settle and heal in order to fully access the situation.  In most instances, the issue resolves itself.  In other instances, when there is a need for a revision surgery, the patient may be held responsible for fees related to materials, anesthesia, operating room costs and/or surgeon fees.
